Subject Access Request (SAR) form
You have a legal right to ask for a copy of the personal information a health or care organisation holds on you. This is known as making a Subject Access Request (SAR). You may also give permission to someone else, such as a family member or your solicitor, to make the request on your behalf.
Your health and care organisation should provide information about how to request your personal information. You do not have to use a form to make your request. However, if an organisation has a form, completing it may help them to find the information you are asking for more easily.
If they do not have their own form, you can complete this template SAR form and check with your health and care organisation who to send it to and whether you can send via email, in the post or drop it off in person.
