Mastercall Healthcare and Dignio Connected Care case study

Mastercall Healthcare is a social enterprise established in 1996. It provides patient-focussed “out of hospital” remote monitoring healthcare, and covers a patient population of around 3 million across the North West of England. Mastercall used the Dignio Connected Care solution to pioneer a technology enhanced living service. This ensures the smooth management of data between patient, carer and clinician. It captures important data from a combination of questionnaires, clinical devices and teleconsultation tools, which allows a clinician to appropriately manage and identify patients who may need further assessment. The platform allows clinicians to respond to changes in a patient’s condition, whilst simultaneously reducing strain on GP practices and emergency service. This helps to ensure that the right patient receives the right care at the right time.

Jacqui Doherty is an advanced clinical practitioner and lead for technology enhanced living for Mastercall.

“A carer recently used our service to relay results from home monitoring equipment to the digital hub on behalf of his wife. Monitoring equipment was left with the patient and her husband, who relayed the results daily to the team in the digital hub. The team then monitored progress and recorded interventions on the Dignio Prevent platform. A little over three weeks later, the patient was discharged from the service. The patient’s husband said that he thought it was a brilliant service.

“The challenge for us was that carers and patients often rely on the interpretation of vague symptoms in their communication with clinical teams. Dignio enhanced our assessments and allowed us to accurately respond to patients remotely, who may have otherwise needed a home visit or a review in A&E

“Deterioration can go unnoticed until it becomes an emergency case, with an increased risk to the patient and a higher cost to the healthcare system. Patients using the Dignio system report feeling less isolated in a time of increased prolonged isolation and social distancing. It’s helped patients who would normally have required a face-to-face assessment to access safe, appropriate and effective health care during these difficult times.”
